Any potential forklift operator must pass this quiz successfully prior to being allowed to do the hands-on practical evaluation portion of the training. The prerequisite training needed must be performed by a MA – 1D Hoisting Licensed person using our materials or other state approved materials before anyone can sit for this quiz. The practical evaluation part of the course will be required every 3 years for each distinct type of forklift being driven (different models, types etc…) but only after the trainee passes this quiz.
