This course is created from media materials created by Foley Services purchased by CHT some time ago. The course is only for CHT supervisors or managers. Usage by non CHT employees is NOT PERMITTED and has been administratively prevented. If you are not a CHT employee and gain access to this class you are violating copyright law and may be subject to fines and penalties.

If you are a CHT employee and a Supervisor or Manager then this course is for you. This class will take about 1 hour for the alcohol portion of the training and 1 hour for the controlled substances portion of the training. There is a short quiz and review of the current paperwork that CHT uses along with site specific procedures which clearly define the actual site process at the end of the training videos.

CHT PIV Forklift In-Service Licensee 3 year Practical Evaluation

This short quiz practical evaluation certificate will be stapled to the actual 3 year evaluation form that has been done. These practical evaluations must be less than 3 years old to be considered valid. An operator must be tested by practical evaluation for EACH TYPE of PIV they will drive. So as an example If you are a maintenance person and want to drive the normal forklifts, the large forklift, the stand-up lift in vacuum furnace area and the walkie trucks we use on the shipping trucks then you would need 4 separate practical evaluations done. Each of these evaluations are good for 3 years and the employee must be re-evaluated on that type device again. if they have an accident or equipment damage prior to the 3 year cycle then they may be required to do the practical evaluation or training dependent on the judgement of the supervisor and/or Vice President. CHT- Facility Vendor or Contractor Safety Process

This course is a simple review of the process County Heat Treat uses to qualify and manage safety for outside companies who will come on site to do work in the facility. It involves understanding the purchasing process and also utilizing the one-page contractor safety checklist to communicate the safety needs and spot audit as nessesary when they are working. All to be sure the outside workers are meeting the minimum safety standards required here. Permanent Contractors or Vendors (like people who work onsite daily) will be assigned the CHT safety training and may be required to take this course as well. It should take no more than half hour to fully complete.

CHT Forklift Personnel Platform Use

This course is designed for those employees who will use the forklift personnel platform including anyone who will be driving the forklift when the platform is being used. Both the forklift driver and the users of the platform are responsible for knowing this safety material before use and after any incident involving this platform. The course is only 10 minutes long and the certificate must be signed off before actual use of the platform.

CHT Powered Aerial Platform

This course is designed for those employees who will use the “Genie” powered aerial platform, it must be completed before use of this equipment is allowed. The course is about 15 minutes long and the certificate must be signed off to be considered completed. Retraining is only required in the event there is an incident documented on the incident reporting system and the powered aerial platform is involved. The user during this incident must be retrained in the cause(s) which were found to be causal in the incident.

CHT Annual Electrical Worker Shock Contact Hold Release / Rescue

This course is designed for those electrical qualified employees who will work on electrical equipment (especially live equipment) on site here at CHT. A trainee must be a qualified electrcial worker for CHT and have the NFPA 70E training prior to taking this annual simple refresher. This course is required one year after initial NFPA 70E training and every year after. The course is about 15 minutes long and the certificate must be signed off to be considered completed. Retraining is required in the event there is a noted incident documented on the incident reporting system and live electrical was involved.

CHT Scissor Lift (Mobile Elevated Work Platform) Training

This course consists of 3 lessons in order. Scissor lift training, Scissor lift Inspection, and Scissor lift Practical Evaluation. The quick quiz associated with this course is in the practical evaluation lesson. The training must be done once before the employee is approved to use the MEWP (scissor lift). The practical evaluations are done initially and then every 3 years OR in the event of an incident / accident for re-training. Overall time for this training should be about 1.5 hours in total not including the apprentice driving time if it is needed. The practical evaluation sheet is considered the certificate for this course. Once it has been done it is completed.

CHT Evacuation Leader Training

This training course is designed for those employees or supervisors who will be responsible for leading the evacuation efforts at County Heat Treat. The will perform as site evacuation leader or departmental evacuation leader. The course is required once before assuming these duties or if any evacuation plan changes significantly affect the material covered in this course.

CHT PIV Handtruck Operator Training & Practical Evaluation

This course is a quick overview of the safe operation of this type PIV which is used on the two shipping trucks and around the shipping area at this time. Drivers who are transporting our products back to customers must use these to maneuver heavy loads in the back of the van trailers. It is expected this whole course including the evaluation to take no more than 30 minutes assuming that the trainee has been apprenticing on the handtruck and is ready to take the practical evaluation.
